Repairs were made and the compressor was started. The flaring was eliminated once all
the compressors were back on line.

Flare Volume: 1281 Assist Gas Volume: 235
This was an unplanned flaring event that occurred when a fuse failed in the PLC. The
fuse failing caused a false level safety high high indication on Oil Reserve Tank: The
false LSHH initiated a full field production shut down. Black smoke was observed
during the initial plant blow down of this incident. The flaring was eliminated after
repairs were made to the PLC and the gas compression facility was restarted.

compressor developed a gas leak on the #4 cylinder gasket. The compressor was shut
down; repaired and brought back on line. No black smoke was flared during this
incident.

The first Flaring event at 00:39 hrs was due to an unplanned shut down of C-SS11A gas lift
compressor. The compressor was shut down to repair of broken sensing line on PSV-5575A.
The second flaring event at 09:21 hrs was due to an unplanned shut down of gas compressor
module. The compressors shut down was due to an automated safety device PSHH on the
discharge of C-5501-A lsr stage.
The third & fourth event was due to an unplanned shut down of the 1WAG compressor. The
compressor was shutdown when a PSHH shutdown alarm in module 55 caused the IWAG
injection gas compressor to shut-down on low suction pressure. Maintenance crews were
dispatched to correct problem. Once IWAG injection compressor was returned to service the
flare was eliminated.
Several hi GOR wells were shut in to mitigate flaring in all four flaring events. The
Compressors were put back on line and the flaring stopped.

This was an unplanned flaring event which occurred when the 08 power turbine was shut-down when a
faulty wire connection tripped the TSHH on the fuel gas manifold of 08 turbine. When this connector
failed it caused the turbine to initiate a fast shut down of the turbine. The automated load shed system
tripped breakers on three gas compressors which sent gas to the flare header.
Maintenance crews were dispatched to correct the problem on 08. Several high GOR wells were shut in
to mitigate flaring. The turbine & Compressors were put back on line and the flaring stopped.

This event was in conjunction with the unplanned shut down of the OS power turbine.
Wells were shut in to mitigate flaring. When wells were being brought back on a gas slug
came through the production separators causing this event. After it passed through the
system flaring stopped.

This was an unplanned flaring event that occurred when work was being performed on
the automation system at C-pad. An erroneous bit was latched causing a production
shutdown of F and L pad as well as the C-pad injection gas compressor (IWAG) and line
heater.
Maintenance crews were immediately sent to the IWAG to restart the unit. No additional
wells were shut in as 2/3 of the field's productivity was shut in already and the flare
volume was the gas in the system working itself through the facilities.
No black smoke was observed during this incident.

flaring during start up. Flaring stopped when the process facility was restarted after the
shut down.

This was an unplanned flaring event caused by an emergency fire and gas shut down on
the PU-0007 power turbine. The event was caused by a failure of three circuit relays in
the fire and gas control panel. The load shed system. shut down C-5501/11A which
caused flaring from the oil separation vessels. Operations and maintenance-staff were
immediately dispatched to make repairs.. Production wells were shut in to mitigate the
flaring. Repairs were made to the fire and gas control system and the power turbine was
restarted. The flaring stopped after the facility was returned to normal operation.

Milne Point's 07 Power turbine shut down due to a failure in a circuit relay inside the fire and gas control panel. When
the relay failed, it activated a fast shut down of the power turbine. Problems with the assist gas system also occurred
and manual intervention was required.
Action Taken to Stop Btack Smoke Flaring:
Several production wells were shut in to mitigate flaring. The assist gas system was manually operated to eliminate the black
smoke during the flaring event. Maintenance personnel made repairs to the 07 power turbine, and assist gas controllers. All of
the process equipment was then returned to normal service.

gas compression facility efficiency became drastically reduced because of the inability to
cool the processed gas. Ambient temperature dropped shortly after this event occurred,
which eliminated the flaring.

This was an unplanned flaring event caused by an unplanned field shutdown -while
performing maintenance on .the HH level switch on the oil reserve tank. While :removing
a hard jumper, a fuse blew creating the field shutdown. The load shed system-shut down-
C-5501/11Awhkh-caused flaring from the oil separation vessels. Operations and -
maintenance staff were immediately dispatched to make repairs... While the field was
being ramped up, the gas compressors had a problem with low suction pressure and shut-
down.. The flaring stopped after the gas facility was returned to normal operation.

Initial Cause for Black Smoke Event: (identify any AOCP permitted equipment, if involved)
Milne Point had a complete field shutdown caused when a fuse failed while performing maintenance on the
instrumentation on the oil reserve tank high high level switch. Problems with the assist gas and gas flowback systems
and low suction pressures on the gas compressors also occurred and manual intervention was required once the gas
facility was brought on line.
Action Taken to Stop .Black Smoke Flaring:
The produced gas flowback system shutdown on low temperature causing the assist gas system to shutdown. Maintenance
and operations personnel identified problem switches and made repairs. Once the gas facility was pressured up the assist gas'
system was returned to service to eliminate the black smoke. All of the process equipment was then returned to normal
service. The black smoke was then elliminated.

This was an unplanned flaring event that occurred when C-pad process module was shut-
down by an ESD when the C-9 production well choke washed out and vented gas into the'
module. IWAG injection compressor was shutdown along with this event causing gas to
vent to the flare tip. Production wells were shut-in to mitigate flaring. Once the incident
was under control the IWAG injection compressor was returned to service and the flare
was eliminated.

Unplanned flaring event occurred when the 1~VAG injection gas compressor was shut
down for repairs. At 2131, C-5531B was taken off line because of a gas leak.
Maintenance crew finished repair of the 1WAG compressor and unit was started to end the
flare event.

This event was an unplanned flaring event that occurred when the IWAG injection
compressor on C-pad shut-down. Maintenance crews were dispatched to correct problem
but were unsuccessful at starting the compressor. Gas lift wells were shut-in to eliminate
flaring.
Time and duration of incidents 22:31 hrs - 1:14 hr
Flare Volume: 155 Assist Gas Volume: 242
This event was an unplanned flaring event that occurred after the ]WAG injection
compressor on C-pad shut-down. Maintenance crews were dispatched to correct problem
but were unsuccessful at starting the compressor. Gas lift wells were shut-in to eliminate
flaring. Further gas rate cuts were made and flare incident was successfully terminated.

Flare events were due to an unplanned shut down of both gas injection compressors
CM-5502/03/31A and CM-5502/03/31B in Module 55. The compressors shutdown
because of high discharge temp. The cause of the high temp was failure of the Interstage
exchanger X-5513. The cooling medium flow was reestablished and the compressors
restarted. During the time of the compressor shutdown a relief valve on the fuel gas
system relieved. The relief valve was reset and flaring stopped. The Injection gas system
was returned to normal operation.

The first Flaring event was due to an unplanned shut down of IWAG compressor. The
compressor shutdown because of an Oxygen mixture problem. The Compressor was
restarted and the flaring stopped. The Second flaring event was cause by the lost of 0701
Power Generation turbine due to the lost of the turbine speed sensor. The Power load
shed scheme worked and shutdown CM-5501/11B Gas-lift compressor and CM-
5502/03/31B Gas injection compressor. After replacing the sensor the Turbine was
restarted . The power was restored to normal capacity. The Compressors were put back

due to unknown problems. Tried to restart IWAG but discovered problem with starter.
Maintenance crews were dispatched to troubleshoot problem and repair starters` High
GOR wells were shut in. Also found PCV-5804 Gas-lift header pressure controller
leaking. Reset the PCV and the flaring stopped off the Gas lift header. The compressor
was returned to service and the flare was eliminated. '

These were planned flaring events that occurred when the IWAG injection compressor
was shut-down due to scheduled maintenance. Maintenance crews had organized several
repairs on the 1WAG which included, installation of a new governor, an exhaust valve
repair on engine, compressor valve replacement, and a oil system vacuum pump
replacement. High GOR production wells were shut-in to mitigate flaring. The overall
project was scheduled for 39 hrs of maintenance but due to effective planning of
maintenance and operations personnel the IWAG injection compressor was down less
than 24 hrs. Once repairs were completed, the gas lift compressor was returned to service
and the flare was eliminated.

Unplanned flaring event occurred when the IWAG injection gas compressor shut down
due to a dead spark plug on #7 cylinder. Maintenance crews were dispatched to change
out the faulty spark plug. Production had been curtailed on February 24`h when wells
were shut in because of bad weather in Valdez, and we were in the process of bringing the
plant backup to full production. This process was halted and no new wells were opened
to production until the problem was corrected. When the compressor was returned to
service, the flare was eliminated.

Flare event due to an unplanned shut down on one of our gas compressors CM-5501/11A
in Module 55. The compressor motor shorted. out one of the electric motor leads. The
cause of the wiring failure is unknown at this time and is being evaluated. High GOR
production wells were initially shut down to mitigate flaring. We then continued shutting
in wells until the flaring was eventually stopped.

Unplanned flare event due to PSV-5564 on Fuel Gas Volume Vessel V-5543 in Module
55 prematurely lifting. This event occurred in conjunction with an offload of source
water from vessel V-5411, which causes V-5411 gas pressure to reach flare pressure
through PCV-5401. When the source water flare event ceased and the flare did not stop,
the operator in Module 55 was notified and found PSV-55641eaking. The PSV was reset'
and the flaring stopped.

packing, and have identified the root cause. High GOR production wells were shut-in to
mitigate flaring. Once repairs were completed, the injection compressor was returned to
service.

in due. to .electrical outage: COl, C03, C21, C23, C26. Also SI high GOR wells C09,
E06, L06 for additional flare relief.
Event turned into unplanned incident during restart attempts at approximately 1500 hrs.
A gas leak was identified on 3rd stage PSV pilot & shutdown was extended to repair leak.
SI following high GOR wells: B09, B22 for further flare mitigation under compressor
RTS at 1900 hrs.
