IreM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. IreM 4b: TPI (Top of Producing Interval). IreM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. IreM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). IreM 20: True vertical thickness. IreM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. IreM 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). IreM 26: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, or Other (explain). IreM 27: If no cores taken, indicate "None". The plans look good. With regard to the BOP test, in the course of completing the planned plugging operations the need for a BOP test will no longer exist so I don't believe any waiver is needed. This sidetrack is currently being drilled by Doyon Rig #14 post the P&A work on the MPU Pesado-O1 leg which occurred on 4/10/07. This sidetracked well will be both drilled and cored to a final well TD of 4440' MD (8-1/2" hole) where open-hole logging data will be obtained. With the objectives of this sidetracked well achieved, the P&A process will begin as indicated below. (NOTE: Comments as related to the recently completed P&A of the Pesado-O1 well are included for reference.) Pesado-O1 ("ACTUAL" Open-Hole P&A on 4/10/07): As there was no casing run into this leg of the well, the Pesado-01 8-1/2" open hole was P&A'ed by spotting 3 "balanced" cement plugs from the final well TD at 4497' 300' back into the 9-5/8" surface casing shoe to 2966' MD (2 - 15.8 ppg G plugs from the well TD back up the-well to 3473' MD and 1 - 17.0 ppg G plug from 3473' MD back up the well to 2966' MD (3rd plug to serve as a kick-off plug for the Pesado-OlA sidetrack)). This P&A plan was reviewed by your office and ample notification was given to the North Slope AOGCC inspector to witness the P&A work as desired. (Note: Chuck Sheave w/AOGCC waived witness to both the pressure test (2500 psi f/15 minutes) and "weight-set" test on top of the top plug with both of these tests occurring on 4/10/07.) Pesado-O1A ("Planned" Open-Hole P&A): 1. A continuous cement plug will be placed in the well from the planned well TD at 4444' ~<« back up the well to 300' above the 9-5/8" surface casing shoe placing the TOC 2955' MD. This 1485' cement plug will be spotted in the well as 3 - 495' balanced plugs with the top plug being weight-set tested OR pressure tested (1500 psi) to confirm the plug placement/integrity. Sufficient WOC time between each plug will be allotted to allow cement to reach thickening time. As currently planned, the 2 bottom plugs will be formulated with 15.8 ppg G cement and the top plug will be formulated with 15.7 ppg Arctic Set as it will develop a quicker compressive strength to accommodate the planned weight-set test or pressure test (1500 psi). The cement volume for these cement plugs will be calculated at hole gauge + 30% OR E- line hole caliper volume + 5% whichever is concluded to be the best hole volume estimate. Also, when setting these balanced plugs, the volume of excess cement circulated out of the well at the calculated cement top for each of the 3 plugs will be noted and recorded in the daily drilling report. With a good weight-set test or pressure test (1500 psi) on top of the upper plug, these cement plugs will effectively isolate all oil bearing zones in the Schrader Bluff and Ugnu formations per AOGCC regulations. With the 3 cement plugs as detailed above in Step #1 in place and "confirmed", this P&A plan includes displacing the 9-5/8" casing from above the TOC at 2955' MD to surface with 9.4 ppg NaCI brine. 3. To complete the down-hole abandonment of this well, a surface cement plug (15.7 ppg AS-1) will be spotted in the top part of the well from the CIBP in the 9-5/8" casing at 250' MD to no deeper than 5' below original ground level. ~~G~ b~LS ~~~~~ 4. From: John Rose, ODE IJ Date: February 13, 2007 Re: MPU Pesado-O1 A Permit to Drill The MPU Pesado-OlA sidetrack is designed to kick-off below the planned 9-5/8" casing point at 3323' MD from the MPU Pesado-O1 parent well. This sidetrack is scheduled to be drilled by Doyon Rig #14 immediately after the completion of the MPU Pesado-O1 test which should place the initial operations date for this sidetrack to 3/30/07. The planned KOP for this sidetracked well will be 3338' where the well will be both cored and drilled to a final well TD of 4192' MD (8-1/2" hole) where again open-hole logging data will be obtained. With the objectives of this sidetracked well achieved, the P&A process will begin.-- That final P&A process for this sidetracked well will include the final P&A requirements for the MPU ; ' 'ano-O1 parent well: 1. Spot/squeeze cement plug(s) (15.8 ppg G) across all cased-hole perforations and/or open- hole hydrocarbon zones (Schrader Bluff/Ugnu) in both the MPU Pesado-O1 and Pesado- O 1 A wellbores. 2. In addition to the case-hole and open-hole P&A work as detailed in Step #1 above, the last open-hole cement plug in the OlA sidetrack (15.8 ppg G) will be brought 100' back into the 9-5/8" surface shoe for the required P&A of that portion of the well. 3. Spot 150' cement surface plug (15.8 ppg Arctic Set) in the top of the 9-5/8" casing. 4. Cut the surface casing and conductor casings below the ground elevation and remove those components. 5. Place Abandonment Marker. 6. Submit a Sundry notice as to the details of the final P&A.
