The patch was pulled on 11/23/15 with LDL(12/2/15)and caliper(11/30/15)diagnostics following. The LDL confirmed the previously patched leak at 6760'with a slight anomaly at 6661' RKB. The caliper data showed competent tubing throughout with slight buckling above the packer. This procedure will outline the steps needed to isolate the selective and return the well to A-Sand only injection. The above-mentioned patch was tested on 10/31 and found to be leaking at the lower packer/snap latch area. This procedure will be to pull and reset the entire patch in an attempt to get a passing MITT and subsequent LDL. Risk/Job Concerns: Max surface pressures(T/1/O)=3000 psi/2000 psi/1000 psi. Min ID above work=2.812"SSSV @ 1822' RKB Fish left in hole @ 6776' RKB. Lower eclipse patch packer(ID 1.75") Last SBHP=2456 psi @ 5940'TVD EMW=7.9 ppg. 4/24/16: Gauged tubing with 2.72" GR to top of eclipse packer at 6776' RKB. 12/2/15: LDL verified leak @ 6760' RKB (anomaly at 6661' ?) 11/30/15: Caliper from 6795' RKB to surface. 11/29/15: Gyro survey 11/23/15: Pulled TTS patch (6753'to 6772' RKB) Steps: Halliburton RELAY/IRS: 1. Perform pre-job safety meeting. MIRU RELAY and support equipment. 2. RIH and pull upper section of patch. 3. RIH and pull lower PIIP packer. 4. Brush and flush patch set area from 6700'to top of eclipse packer at 6776' RKB. (Ensure ID through eclipse packer is clear prior to setting patch. ID of eclipse packer is 1.75") 5. Set lower PIIP @ 6771' RKB in pup joint below GLM @ 6761'. (Previous settings of the lower packer were 6772 and 6774. Try to avoid setting the packer at the same depth.) 5. RIH with upper PIIP and 14'spacer pipe stabbing into lower packer, setting upper packer at 6757' in 8' pup joint above GLM. 6. Set TTS test tool in upper packer and MIT-T to 2500 psi. A) Passing MIT-T- Pull test tool B) RIH with slim LDL tools and verify competency of snap latch and lower patch by logging LDL across patch. If patch looks good RDMO and turn well over to BOL. If lower assembly is leaking, pull upper section and reset. 7. If MIT-T to 2500 psi on upper packer fails, POOH and pick up LDL string. 8. Run LDL to confirm upper packer is holding and to investigate possible anomaly @ 6661' RKB. " A) If second leak is identified above patch, RDMO for second patch procedure. 9. Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped January 18th and January 24th, 2009. The corrosion inhibitor/sealant was pumped February 21St through February 23`d , 2009. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Please call MJ Loveland or Perry Klein at 907-659-7043, if you have any questions. Sincerely, i~ ~~~~ C M.J. Loveland ConocoPhillips Well Integrity Projects Supervisor ConocoPhillips Alaska Inc. As the attached AOGCC Safety Valve System Test Report indicates I witnessed the testing of 11 Wells and~22 components with three failures observed. The Iow preSsure Pilots on wells 2U,5, 2U-14 and 2U-16 tripped well below the required set Point. These three pilots Were adjusted and retested at the time of this.test. In additiOn to the three failed pilots several other pilots tripped very near the minimum pressure, these were reset and operated'-prOperly when retested. The 13.6% failure rate will require a retest of this Pads Safety valve System in 90 days rather than the normal i/l~%day cycle. Smith Alaska Oil and Gas Conservation Commission 3001 Porcupine Street Anchorage, AK 99501 Dear L. C. Smith: Per our conversation on September 4, 1985, ARCO Alaska, Inc. proposes to convert 110 production wells to water injection to implement fullfield waterflood of the Kuparuk River Unit. As discussed a single "blanket" sundry notice (form 10-403) has been completed for all well proposed to be converted to water injection. Individual subsequent sundry notices will be filed when the actual well conversions occur. As stated in form 10-403, well conversion to water injection will occur in two primary stages. The first stage will convert approximately four wells per drillsite to achieve a nine-spot pattern. First stage conversions are expected to be completed by approximately 10/15/85, prior to waterflood startup. The second stage of conversions will convert approximately four additional wells per drillsite to achieve a line drive pattern. Second stage conversions are expected to be completed mid-December to January 1986. I would like to express my thanks and appreciation to the Commission in working with ARCO Alaska, Inc. in this area. If you-have any questions please call me at 263-4212. Sincerely, T. M. A 20", 2000 psi annular diverter will be installed on the 16" conductor while drilling the surface hole. The 9-5/8" casing will be set through the West Sak Sands at approximately 3121'MD (3026'TVD). A 13-5/8", 5000 psi, RSRRA BOPE stack will be installed on the 9-5/8" surface pipe. It will be tested upon installation and weekly thereafter. Test pressure will be 3000 psig (1800 psi for annular). Expected maximum surface pressure is 2661 psi. Pressure ca]cu]ations are based on latest reservoir pressure. Surface pressure is calculated with an unexpanded gas bubble at surface, including temperature effects. Selected intervals will be logged. The well will be completed, using the workover rig, with 3-1/2", 9.2#, J-55, EUE 8rd tbg. Non-freezing fluids will be left in uncemented annuli through the permafrost interval. Selected intervals will be perforated within the Kuparuk formation and reported on subsequent reports. A downhole safety valve will be (see attached sheet) 23.
